Kossuth Regional Health Center’s patient portal gives you easy, online access 24 hours a day, to portions of your hospital medical records and test results.
What can I do with my Patient Portal:
- Request an appointment time and review your scheduled appointments online
- Receive your test results faster than through the mail
- Ask your provider a question when it’s convenient for you
- You will only receive an e-mail to alert you that information has been sent to your portal account. You will not receive junk emails and your e-mail will not be shared

Enroll in person – Patients may also ask registration staff at Kossuth Regional Health Center to send them an invitation to the portal. Please bring a photo ID to begin the registration process. A valid email address is required to use the patient portal.
Parents & Patient Portal Access
Due to government regulations in regards to patient privacy, at age 12 youth have the right to deny portal access to parents and guardians. Parents may continue to have access to their child/dependent’s portal after age 12 if the child signs a Designation of Patient Portal Access Form.

For electronic records for hospital services prior to August 31, 2015, please visit here. Records from this timeframe are only available to those who created an account with My Health Record prior to the transition to the Patient Portal.

What types of apps can I link my health record to?
You can connect and pull in data from your health record to the applications listed below if you choose. Please check back here for the most recent list of apps available.

- TidyHealth (This app does require the patient to download their health record and import into TidyHealth)
These apps give you the ability to view some of your medical record (or someone else’s records, where you have proxy rights) on an app or website other than your patient portal.
