Immunizations

Community Health provides child and adolescent immunizations throughout Kossuth County for newborns through 18 years of age. 

If you or your child have recently received a vaccine, learn more about it from the list of Vaccine Information Statements, provided by the Centers for Disease Control and Prevention.

Vaccines For Children

  • Vaccine available to newborns to those 18 years of age 
  • Medicaid/Title 19 enrolled or
  • No health insurance or
  • American Indian or Alaskan Native or
  • Underinsured (no immunization coverage, covers only certain vaccines or caps the cost at a certain amount)
  • Unmet deductibles or co-pay do NOT qualify persons for Vaccines for Children

If you have insurance, please contact your insurance company to determine if your insurance covers immunizations or includes them under the category of preventative care. If insurance will pay, please call your health care provider’s office to receive vaccinations.

If your insurance doesn’t cover immunizations or for more information about immunizations, please contact Community Health at 515.295.4430 and or 1.800.603.8433 to arrange a time for your child to receive immunizations.

Immunization & Flu Clinics

Don't Get The Flu...Get The Flu Vaccine

Protect yourself before the seasonal flu strikes by getting your annual flu vaccine. Children who receive their immunizations in Community Health through the Vaccine for Children program can receive their flu vaccine in Community Health.

 

Vaccination is especially important for children, adults age 65 and older, pregnant women and people with asthma, diabetes and other long-term conditions who are at high risk from flu complications.

School Based Vaccine Clinics

Community Health staff typically visit schools throughout Kossuth County to offer student flu vaccine clinics in September and October. In order to participate, students need to bring a completed flu consent form and copies of a health insurance card (both sides) by mid September. Call Community Health for this year’s deadline for consent forms. The school-based clinics are the only vaccine clinics that will be using a paper consent form. All other registrations will be done with KRHC’s electronic health record system.

Flu Vaccine Information

Please bring a copy of your insurance or Medicare card for insurance to cover the vaccine, or be prepared to pay the fee. For more information and updates call KRHC at 515.295.2451.
