Kossuth Regional Health Center

Patient Rights & Responsibilities
Kossuth Regional Health Center recognizes the following patient rights and responsibilities:
- All patients have the right to be treated with kind and considerate care at all times.
- Patients can expect reasonable safety at Kossuth Regional Health Center, and to be free of physical, verbal, sexual or mental abuse.
- Patients have rights concerning the use of individually identifiable health information, according to a federal law named the “Health Insurance Portability and Accountability Act” (HIPAA). View our Privacy Practices in full.
- Patients have the right to request exclusion from the hospital directory. Patients who are included on the directory will be able to receive deliveries of cards and flowers sent to that patient. The patient’s name will appear on a list for clergy members of his or her faith (if applicable).
- All patients have the right to be informed of alternative treatments and to choose among the alternatives, including the right to refuse treatment to the extent permitted by law, and to be informed of the medical consequences of their actions. All patients are responsible for their own actions if they refuse treatment or do not follow the doctor’s recommendations.
- All patients have the right to every consideration of privacy concerning their medical care program. Patients are responsible for being considerate of the privacy of other patients. Telephones, televisions, radios and lights should be used in a manner agreeable to others.
- All patients have the right to continuity of care, transfer, and consultation with other medical specialties.
- All patients have the right to examine and receive an explanation of their bill, regardless of the source of payment. Patients have the responsibility to provide information necessary for claim processing and to be prompt in payment of their bills.
- All patients have the right to know the rules and regulations that apply to patient care and conduct and are responsible for following those rules and regulations.
- All patients have a right to receive an explanation of their treatment program and to ask for further clarification if the course of treatment is not understood.
- Patients have the responsibility to cooperate in their treatment program.

Well Child Information Sheets
A printed information sheet for the appropriate age will be provided at your child’s well child visit. Information sheets can also be accessed here:
For Athletes
Every year each student (grades 7-12) shall present to the student’s superintendent a certificate signed by a licensed provider to the effect that the student has been examined and may safely engage in athletic competition.
Minor Consent Form
The purpose of this form is to obtain guidance from the patient’s parent or legal guardian, to treat a child if he or she requires routine or non-acute medical care while you are unable to attend the appointment or be reached by phone.
Advance Directives
If you are 18 or older and mentally competent, you have the right to make decisions about your medical treatment. If you want to control decisions about your health care, even if you are unable to make or express them yourself, you will need an Advance Directive. An Advance Directive is a set of instructions you give about the health care you want in the event you lose the ability to make decisions for yourself.
There are three kinds of Advance Directives:
- Living Will
- Health Care Power of Attorney
- Advance Instructions for Mental Health Treatment.

Other Amenities
Pastoral Care
Pastors of all denominations are welcome to visit patients at any time. Hospice of the Heartland also offers a hospice chaplain for those who would like a visit.
Kossuth Regional Health Center offers a chapel for use by patients, guests and staff. The chapel is located on the first floor of the hospital, down the hall from the elevator.
Meals
We are pleased to offer room service dining to our patients. All patients are invited to order their breakfast, lunch, and dinner from KRHC’s room service menu. If you are on a restricted diet or have questions regarding the menu, the dietary staff will be glad to assist you with your order.
Tobacco-Free Campus
Kossuth Regional Health Center is a tobacco free campus. This policy applies to all staff, patients and visitors and includes cigarettes, chewing tobacco and all other forms of tobacco. All tobacco products are prohibited within the KRHC facility, outside of the facility, in the parking lots, within personal vehicles parked on KRHC grounds or in KRHC vehicles.
Language Line
Clear communication is important, especially when making health care decisions. If interpretation sevices are needed, patients may request the LanguageLine. This program offers online translation services in a variety of languages.
LanguageLine is available to all departments of Kossuth Regional Health Center – just ask any staff member to get started during your visit or when calling in for an appointment.
